How Grip Strength Became A Useful Measure
Grip strength predicts a range of outcomes not because hands matter especially, but because it is a cheap proxy for total muscle function that is easy to measure consistently.

Grip strength appears in research on ageing, recovery and general health far more often than any other single physical measure. Its usefulness comes from being a proxy rather than from the hands themselves.
It stands in for whole-body muscle function
Muscle strength tends to decline together across the body, so a measurement at one site tracks the overall picture reasonably well.
Grip correlates with strength in the legs and trunk, which are far harder to measure safely and repeatably in older or unwell people.
The hands are also relatively unaffected by the joint problems that make lower body testing painful or impossible for many of the people most worth assessing.
The measurement is unusually consistent
A handheld dynamometer produces a number in seconds, requires almost no space and needs minimal training to use.
Protocols specify posture, elbow angle and number of attempts, which makes results comparable between clinics and across studies.
Consistency is why the measure spread so widely. Many better measures exist, but few can be taken identically in a clinic, a home and a research setting.
Muscle mass and function are related but distinct
Loss of muscle mass with age is accompanied by a disproportionate loss of strength, so mass alone understates the functional change.
Grip captures function, including the nervous system's ability to activate muscle, which imaging of muscle size does not.
This is why it is used to define sarcopenia alongside measures of mass, rather than being treated as interchangeable with them.
Association is not the same as cause
Low grip strength is associated with slower recovery from illness, longer hospital stays and reduced independence.
That does not mean training grip changes those outcomes, since grip is reflecting overall condition rather than driving it.
Squeezing a hand gripper would raise the measurement without altering what the measurement was standing in for, which is a general hazard of proxy measures.
What actually moves the underlying thing
Whole-body resistance training addresses the muscle function that grip reflects, and adequate protein intake supports it.
Improvements in strength are achievable at any age, including in people who are frail, which is one of the more consistent findings in the field.
Where strength has declined noticeably, an assessment can identify contributing causes, and a physiotherapist can set a programme suited to the individual.
